Letters & Documentation
Clients sometimes need written documentation of their counselling: most often for an insurance claim, occasionally for an employer or educational institution.
As these documents fall outside the scope of a counselling session, they are charged separately and prepared in the order requests are received. Each one involves reviewing your file, checking dates, and drafting language appropriate for a third party to receive.
Requests are made by email. I will confirm the fee and completion date before any work begins.

Time Chit: Confirms your name, date and time of a single appointment attended. Issued at the end of the session (please provide notice).

Duplicate/reissued invoice or receipt: A duplicate copy of an invoice already issued to you. Identical in content to the original.

Letter of Attendance: Confirms the date and time of your appointment, type of service provided, and my professional registration.

Counselling Verification Letter: Sets out dates of appointments attended, number of sessions over a stated period, the general nature of the concerns you brought, and my scope of practice and limitations. Issued with your written consent.

Supporting Letter: A letter to an employer or institution supporting a specific request, such as no-pay leave (NPL). Provided where I am able to support the request.

Completion of an insurer or employer form: Filling in sections of a form that fall within what I can properly complete. Fee confirmed before starting, depending on length of documentation.​
Disclaimer for documentation: These are records of attendance and records of counselling provided, not assessment reports. As a Registered Counsellor (SAC, Reg. No: C0940) I do not provide diagnoses, so if your insurer requires a formal assessment or an opinion on your fitness for work, you will need to see a psychiatrist or clinical psychologist.
